Forms to Print for Baptism Registration

• Baptism Registration Form

     As this form is completed             please use legal names or names as they appear on the child's birth certificate.

• Baptismal prepartaion Completion Certificate

 Upon completing the online  class, a certificate will be available for printing. This certificate is  required  to register your child for Baptism. 

• Godparent - Sponsor Eligibility Form

        This form must be completed and signed by the godparents' church.
